1. Pages 2 – 4 Type directly into this portion electronically or print these pages and handwrite.

2. Print pages 2-12

3. Pages 5 – 12 Please handwrite all appropriate information.

4. Include a $50 non-refundable application fee (payable to Brebeuf Jesuit).

5. Please send the ‘Records Release Form’ (page 12) to the applicant’s current school, and they will forward the appropriate information to us (6th, 7th and 8th grade transcripts and standardized test scores.).
